CI: Advanced arteriosclerosis. Hyperthyroidism.Hypersensitivity. Glaucoma. Agitated states.History of drug abuse. Pulmonary HTN or severehypertension. Heart murmur or valvular disorders.Emotional instability. History of psychiatric illness.Concomitant use w/ other anorectic agents. MAOItherapy or w/in 2 wk of discontinuation. Pregnancy& lactation. Child & elderly.
SP: Increased risk of developing primary pulmonaryHTN (PPH). HTN & severe CV disease. Prolongeduse may induce dependence w/ withdrawalsymptoms on cessation of therapy; do not use for more than 3 months. May increase convulsions insome epileptic patients. Toxic psychosis. Pepticulceration. Prostatic hyperplasia.
AR: CV & CNS disturbances; GI effects; allergicreactions; impotence; changes in libido,gynecomastia, menstrual upset; hematopoieticeffects; dyspnea, hair loss, muscle pain, dysuria,increased sweating, polyuria.

CI: Acute alcoholism; convulsive disorders; headinjuries; increased intracranial pressure; comatosepatients; resp depression & obstructive airway
disease; patients on established opioid agonists.SP: Hepatic or renal disease; pregnancy, lactation;hypothyroidism; adrenocortical insufficiency;asthma; prostatic hyperplasia; shock; hypotension;inflammatory or obstructive bowel disorders;myasthaenia gravis; infants/neonates. Reduce dosein elderly & debilitated patients. May precipitatewithdrawal symptoms in narcotic addicts.
A R: Sedation, nausea, dizziness, vertigo,hypotension, miosis, headache, hypoventilation,resp or CNS depression; tachycardia, bradycardia,urinary retention, coma, vomiting, drowsiness,sweating, confusion, dry mouth, diaphoresis,withdrawal syndrome.
DI: Plasma-buprenorphine concentrations may beaffected when co-administered w/ drugs that
induce or inhibit cytochrome P450 isoenzymeCYP3A4. Enhanced depressant effects w/ other CNS depressants e.g. alcohol, anaesthetics,anxiolytics, hypnotics, TCAs & antipsychotics.Diazepam may produce resp & cardiac collapse.
CATHINE(D-NORPSEUDOEPHEDRINE)
I: Obesity.
CI: Patients treated w/ MAOI or stopped MAOI w/inthe last 2 wk; moderate or severe hypertension;pulmonary hypertension; structural cardiacabnormalities or advanced arteriosclerosis; adrenaltumour; hyperthyroidism or hyperexcitability; coronarythrombosis; closed-angle glaucoma; pregnancy.
SP: Heart disease, mild hypertension, CHF or anginaof effort; prostate hyperplasia; patients w/ historyof drug or alcohol abuse; DM; psychotic patients; renaldisease. Growth retardation may occur in children.

DI: Reduced effect of guanethidine. Increased riskof arrhythmia in patients treated w/ digitalis due toits sympathomimetic effects. Increased risk of arrhythmia w/ chloroform, cyclopropane, halothaneor other halogenated anesthetics. The effect of cathine may be reduced/enhanced by tricyclic anti-depressants. Cathine should not be used in patientstreated w/ a MAOI or w/in 14 days of stopping MAOIas hypertensive crisis may occur if used.
EPHEDRINE
I: Relief of moderate to severe acute asthma, relief of broncoconstiction.
CI: Hypersensitivity. Hypertension, thyrotoxicosis,BPH. Lactation.
SP: Ischaemic heart disease, hyperthyroidism,diabetes mellitus, hypertension, angle-closureglaucoma, renal impairment; prostaticenlargement; pregnancy, elderly.
AR: Anxiety, tachycardia, tremor, dry mouth,hypertension, cardiac arrhythmias, impaired
circulation to the extremities, nervousness, insomnia,palpitations. Difficulty in micturition in patients w/prostatic enlargement. Nasal drops: Local irritation,rebound nasal congestion & drug-induced rhinitis onprolonged use. Delusions, hallucinations. Seen w/hypersensitivity & overdosage. Acute CNS & CVSstimulation presenting as vomiting, fever,hypertension, psychosis. Cardiac arrhythmias.
DI: Reduces antihypertensive effect of bethanidine& guanethidine. May increase clearance of dexamethasone. Increased incidence of adverseeffects when used w/ theophylline. Severe HTN whencombined w/ MAOIs or within 2 wk of discontinuanceof MAOI treatment. Increased risk of arrhythmias w/cardiac glycosides, quinidine or tricyclicantidepressants. Increased vaso-constriction or
pressor effects w/ ergot alkaloids or oxytocin.
FLUNITRAZEPAM
I: Induction of anaesthesia. Premedication insurgery. Short-term management of insomnia.
CI: Preexisting CNS depression or coma, respdepression, acute pulmonary insufficiency or sleepapnoea; severe hepatic impairment; chronicpsychosis, phobic or obsessional states; mayprecipitate suicide or aggressive behavior; not tobe used alone to treat depression or anxietyassociated w/ depression; glaucoma. Pregnancy,lactation. Neonates.
SP: Chronic pulmonary insufficiency; elderly or debilitated patients; muscle weakness, impairedliver or kidney function; drowsiness may affectskilled tasks; monitor cardio-respiratory functionwhen used for deep sedation; personality disordersor organic brain changes; history of alcohol or drugaddiction; resp depression & hypotension w/
parenteral admin; this route should only be usedwhen facilities for reversing resp depression areavailable.

May impair ability to drive or operatemachinery. Monitor cardiac function in patientsw/ HTN or cardiac decompensation. Pregnancy& lactation.
AR: Emergence reactions e.g. vivid dreams,hallucinations, confusion, irrational behaviour.Increased muscle tone sometimes resemblingseizures. Temporary HTN & tachycardia.Hypotension, bradycardia, arrhythmias.Respiratory depression, apnoea, laryngospasm,diplopia, nystagmus, nausea, vomiting,lachrymation, hypersalivation, raised intraocular & CSF pressure, skin rash & pain at inj site.

AR: Urticaria, GI upsets, palpitations, tachycardia,elevated blood pressure, overstimulation,restlessness, insomnia, tremor, headache, drymouth, dizziness, dysphoria, euphoria, psychosis,
changes in libido & impotence. Primary pulmonaryHTN & valvular heart disease.
DI: Decreased excretion by antacids, carbonicanhydrase inhibitors (e.g. acetazolamide,dichlorphenamide, methazolamide). Excretion isincreased by urinary acidifiers. Increased risk of adverse effects w/ other sympathomimetic agents.There is a risk of arrhythmias when used w/ TCA.May reduce serum concentrations of phenytoin,phenobarbital and ethosuximide. Severehypertensive episodes w/ MAOI. Primarypulmonary HTN or valvular heart disease mayoccur when phentermine is used concurrently w/fenfluramine or dexfenfluramine. Avoid concurrentuse w/ sibutramine.

Tissue necrosis &sloughing; myocardial & arterial necrosis.
DI: Increased risk of HTN & arrhythmias if givenw/ cardiac glycosides, quinidine or TCAs.Increased risk of vasoconstrictor effects if givenw/ ergot alkaloids or oxytocin.
