Patients are most at risk for readmission within the first 24 

hours post‐discharge

Nearly 1 in 5 Medicare patients 

are readmitted within a month of discharge

Fire departments approached Park Nicollet, stating around 70% of their calls are now medical– with many 

patients right out of the hospital.
