Instructions for the use of logbook
Aim of the logbook
The purpose of the logbook is to provide one source of evidence for the or-
thopedic scientific council that you have attained the desired level of compe-
tency required for licensure. It is the place where you are going to document
experiences and operations you performed during your training.
The logbook is divided into several sections. These instructions will help you
completing those sections correctly.
Personnel information
Please fill in all your personnel information required . This will help the
Egyptian Fellowship Administrators to process your logbook during scien-
tific council evaluation half annually and finally 6 months before sitting for
the final exam. Your personnel photo should be attached to the logbook and
you should sign the personnel information page
Clinical Cases and outpatients clinics
In addition to your general orthopedic training, You will rotate in several
subspecialties as clarified in the curriculum. During your rotations you will
attend various outpatients and operative theatre experiences.
Outpatients clinics
For each training year, You will find tables to record your outpatients atten-
dance experiences. For each attendance write the following information:
1. The type of the clinic attended
2. The date
3. The location (which hospital & specialty)
4. The number of patient examined and number discussed with trainer
5. Signature and clear name of attending consultant or trainer

Inpatient’s rounds
For each training year, there are tables to enter data relevant to your clinical
rounds. For each clinical round you are requested to register the following data:
Date of round
Number of patients seen and number of cases discussed
Your role (participant, presenting cases or leading the round)
Trainer/Consultant clear name and signature
Operative core competence
For each training year you will find details of operative core competencies and
tables to register the type of achieved competency, date and your trainer signa-
ture.
List of required competencies are available in the curriculum and training book.
Surgical procedures (operative log)
The requested surgical procedures and your expected level of participation are
available in your curriculum. For each year of training, you will find tables to
register the surgical procedures you participated in and your level of participa-
tion. The key for trainee participation in surgical procedures is:
O: Observer
A: assistant
PS: Performed under supervision
PU: Performed Unsupervised
T: Training junior trainee
Your trainer should sign each individual procedure to confirm your participa-
tion.
Educational activities
Educational activities must be documented in the logbook and these in-
clude: lectures, journal clubs, scientific meetings (morbidity & mortality,
radiology and clinical meetings) , conferences (national/and/or interna-
tional) , workshops attended, presentation delivered by the trainee during
training, online CME, research activities and examination taken
Any attended activity must be signed

Annual operative summary table
At the end of each training year, you are requested to provide documented
summary of all operative activities you participated in as assistant or first
surgeon. The tables are present in the last page of each year log and should
be signed by your trainer and educational supervisor
Assessment of logbook activities
1. Your trainer will assess your logbook weekly for completion and provide
feedback
2. Your educational supervisor will assess your logbook monthly, provide
verbal or written feedback and counter sign important activities
3. The HARP committee will revise your logbook:
A) Half annually
B) Six months before the final exam
To be noted that unsatisfactory completion of the logbook would lead to
HARP 2, 3 or 4. You are legible to sit for part 2 exam on reception of
HARP 5.
Important Notice:
It is your responsibility to maintain accurate and completed logbook and to
regularly update your records. Shall you meet any difficulty; you must
contact your trainer or your specialty administrator at the Egyptian
Fellowship Board.
