Qualified Person’s Statements and CertificationsAs the qualified person, I certify that:
A phase one environmental site assessment of the RSC property, which includes the evaluation of the information gathered from arecords review, site reconnaissance, interviews, a report and any updates required, has been conducted in accordance with theregulation by or under the supervision of a qualified person as required by the regulation.
As of 2015/04/09, no phase two environmental site assessment is required by the regulation for theRSC property and based on the phase one environmental site assessment for the RSC property, in my opinion, it is notnecessary for any other reason to conduct a phase two environmental site assessment for the RSC property.
As of 2015/04/09, in my opinion, based on the phase one environmental site assessment, there is noevidence of any contaminants in the soil, ground water or sediment on, in or under the RSC property that, if the RSCproperty were put to any of the types of property uses listed in subsection 1 (2) of the regulation, are likely tointerfere with any of those types of property uses.
I am a qualified person and have the qualifications required by section 5 of the regulation.
I have in place an insurance policy that satisfies the requirements of section 7 of the regulation.
I acknowledge that the RSC will be submitted for filing in the Environmental Site Registry, that records of site condition that are filed inthe Registry are available for examination by the public and that the Registry contains a notice advising users of the Registry who havedealings with any property to consider conducting their own due diligence with respect to the environmental condition of the property,in addition to reviewing information in the Registry.

PHASE ONE CONCEPTUAL SITE MODEL
The Phase I ESA Conceptual Site Model (CSM) showing the physical features of the Phase I study
area is presented on Figure 1. No APECs are present within the Phase one Study Area. A summary
of the CSM is provided below.
Site Features: The site is located near the northwest corner of Old Kingston Road and Fairwood
Crescent in Toronto, Ontario. The site is approximately 0.19 hectares (0.47 acres) in size, and is
generally rectangular in shape having a PIN # 06497-0059 LT, approximately 24 m by 56 m,
located at the northeast corner of the Old Kingston Road and Fairwood Crescent in the City of
Toronto. The site is relatively flat with a subtle slope to the southeast. Drainage of the site is
generally to the southeast and localized towards adjacent roadways along the property boundaries.
The Phase One Property consists of a single storey with a basement Community Centre/Place of
Worship and some parking areas.
Geology/Hydrogeology: Based on the surface geology map located at the Geological Survey of
Ontario website, indicates that the Phase One Property is underlain by upper Ordovician formation
which consists of shale, limestone and siltstone. The topographic map located at Ontario Ministry
of Natural Resources website indicates that the topographic gradient at the Phase One Property
generally descends from the east to southeast towards the Highland Creek.
Potentially Contaminating Activities (PCAs): The following PCAs have been identified off-site
within the Phase One study area (all down-gradient):
_ Salvage yard including automobile wrecking (78 m to the south of the Phase One Property);
_ Gasoline and associated products storage in fixed products (210 m to the east);
_ Pharmaceutical manufacturing and processing & Gasoline and associated products storage in
fixed products (9 properties. All down-gradient);
_ Transformer manufacturing, processing and use (210 m to the east);
_ Gasoline and associated products storage in fixed products (110 m to the southeast);
_ Transformer manufacturing, processing and use (2 properties. Both down-gradient);
_ Vehicles and associated parts manufacturing (approximately 198 m to the east);
_ Gasoline and associated products storage in fixed products (5 records of spill. All down-gradient
and more than 110 m from the Phase One Property);
_ Gasoline and associated products storage in fixed products (110 m to the southeast);
_ Gasoline and associated products storage in fixed products (one business registered under the
RSC database, located 42 m to the south of the Phase One Property);
Areas of Potential Environmental Concern (APECs): As a result of these off-site PCAs (all
down-gradient), we have not identified any PCAs that may affect the Phase One Property.
Therefore, no area of potential environmental concern was identified at the Phase One Property.
Contaminants of Concern: There are no contaminants of concern associated with the site.
Water Wells: Based on the information obtained through MOECC Computer Print Out Data Well
Search, there are no drinking water wells located on the RSC property and within the Phase One
Study Area.
Migration Pathways: In general, potential migration pathways for sub-surface contaminants at the
site would consist of buried utilities or naturally occurring fractures in the shale. Drainage lines,
which have not been mapped, may convey contaminants to the ground. Due to low potential for
contamination to be present at the site, the existing underground utilities have no material effect
for potential contaminant distribution and transport.
Uncertainty: The main uncertainty with the CSM is the lack of subsurface investigations at the
subject site. Despite on some uncertainties, the validity of the Phase One CSM is not affected due
to low potential for soil and groundwater at the site being affected by potential contaminants.
WHETHER PHASE TWO ESA REQUIRED BEFORE RSC SUBMITTED
Based on the records review, interviews and site inspection conducted for this Phase One ESA,
MEC Global concludes that no APECs are present at the site. Therefore, a Phase II ESA is not
required in order to file a Record of Site Condition (RSC) for the property under O. Reg. 153/04
as amended by O. Reg. 511/09, further amended by O. Reg. 245/10, and O. Reg. 179/11.
Designated substances such as asbestos, lead, silica, mercury, as well as other regulated substances
such as ODSs are suspected to be present in the building materials and equipment of the structure.
The buildings appeared in good condition and these suspected substances do not pose an
immediate risk to its occupants. However, if the structures are to be renovated or demolished, a
designated substance and hazardous materials survey should be conducted to ensure the health and
safety of the workers.
RSC BASED ON PHASE ONE ESA ALONE
Since no APECs are identified above, a Record of Site Condition can be filed based on the Phase
One ESA alone.
