What information is needed in the Support Plan?

•All about you?•What is important to you?•What you want to make your life better, or change?•What you want to do?•How you are going to get the help you need?•How you will spend your budget?•How you will manage your support?•How you will stay in control of things?

Example A

Example BMy name is John Smith and I am 64 years old. I have been married to Elizabeth for 45 years and we have three children and seven grand children. They are my life. I have had COPD for about 12 years now and nothing is the same. I want my Personal Health Budget to help me improve my quality of life, not to cure me as I know that is not possible. I know that my medication and oxygen will have to stay, but I would like to get out more and get some fresh air – maybe on the sea front and even better, to be with my grandchildren. Maybe I could use my budget to buy a scooter that could also carry my oxygen, after all I only live about half a mile from the sea front – I might even get a pint with my mate Tommy who I used to work with at the steelworks! Surely I would get more out of that than seeing someone about my depression?

Any Questions?
