CPRS ORDERING FOR BLOOD BANK TESTINGAND BLOOD COMPONENTS

Under your patient’s profile, select “Orders” at the bottom of the screen followed by “Add New Orders” on the left side.

Select 47- BLOOD TRANSFUSION

Select “Blood Bank Request” at the top of the “Blood Transfusion Screen”

In the “Diagnostic Test” drop down box, select the desired test.

Select the desired collection type and collection date/time for the testing.

If blood products also need to be set-up for the patient, select the desired blood component and quantity at the top right portion of the Blood Component and Diagnostic Testing Form. Please include an accurate indication for transfusion or reason for request.

Sign the order by clicking “Action”: Signature on Chart…

When placing an order, the “Patient Information” tab of the Blood Component and Diagnostic Test Order Form window shows if there is a current T&S, and if so, when the sample expires. Units of blood that are available for issue are also displayed. If the T&S specimen is still active, you only need to order the blood components to be set up under blood bank requests. No diagnostic testing needs to be selected with this order.

In order to view previous blood bank testing performed at the Durham VAMC, select “Labs” at the bottom of the screen and then select “Blood Bank” on the left side. Here you are able to view the patient’s historical ABO/Rh, previously identified antibodies, previous transfusion reactions , and other diagnostic testing previously performed including: ABO/Rh, antibody screen (ABS), and DAT.

If a patient has not had an ABO/Rh performed since 3/7/2009, an additional ABO/Rh needs to be performed in order to confirm your patient’s blood type prior to crossmatching/setting up blood products. The screen above shows where the ABO/Rh test is located in the order form.

If RBC’s are ordered on a patient that does not have an active T&S, you will see this error message. The blood bank cannot crossmatch RBC’s until a T&S specimen and received and testing is complete. A current T&S is not required to set up Platelets, FFP, or Cryoprecipitate. It is only required that a T&S has been performed in the last 12 months. For example, if a T&S was performed on a patient on 4-4-12, we are able to set up PLTS, FFP, or CRYO until 4-4-13.

If a test or blood product has been ordered in error, you may remove it from the order here.

If the emergency release of blood products is necessary, please call the blood bank (extension 6532) to notify staff. Give the technologist the patient’s full name and SSN, and immediately place the order in CPRS. The order is placed the same way as other blood component orders, with “EMERGENCY RELEASE” in the comment box. The blood bank cannot release the blood until we have this order.
