Pressure Ulcer Measures

K-HEN recommended measures• Outcome: Patients with significant

(Stages III & IV) hospital acquired pressure ulcers - (rate per 1,000 discharges) (#61, #62, or #63 depending on your patient population)

• Process: Skin assessment documented within 24 hours of admission and daily thereafter (#57)
