What we have learned:

We are not alone.

What we have learned:

Safety is not top of mind.

Every 13 minutes and 14 seconds a patient dies in Canada from preventable harm in healthcare

Patient safety incidents rank thirdbehind cancer and heart disease

What we have learned:

Learning from failure is an opportunity that is often missed.

What we have learned:

There are a lot of good people working hard in the “system.”

“Every system is perfectly designed to get the results it gets”

What we have learned:

Technology isn’t the hurdle… or the silver bullet.

What we have learned:

There are pockets of innovation but not evenly distributed and it doesn’t spread.

What we have learned:

There is a gap between the exciting initiatives that we hear about and the patients stories that are shared with us.

What we have learned:

Communication and flow of information are big problems.

What we have learned:

Status Quo is not OK.

Some people are ready for change and want to be a part of it.

So now what?

Think big – Dream forward –We need to do it together

• Imagine if patients (and their families/loved ones) were considered part of their care team

• Imagine if everyone understood the role they could play in the future of our healthcare system

Team Scheme

Teamwork vs Group work

Teamwork makes the dream work

• Everyone has a role and diverse perspectives are required

• Cultural/mindset shift is needed• Teams evolve over time but

everyone must understand who is on the team and who is leading at any particular moment

• Strengths and potential weaknesses are understood

• Team members are utilized to their maximum potential and gaps are filled when needed

Innovation & Impact

• Innovation is about doing things differently

• Innovation may be an improvement but simply improving things isn’t innovation

Role of technology & digital health

• Communication and information flow are critical

• Everyone on the team needs access to information to make safe and informed decisions

• Tools to be utilized to maximize team potential

How do we get there?

• Patient Safety always needs to be top of mind

• Systems thinking – big picture• Information architecture and tools

that enable the flow of information to all team members

• Teamwork mindset is the foundation

“Optimism isn’t a belief that things will automatically get better; it’s a conviction that we can make things better.”

– Melinda Gates
