Group Insurance Board (GIB) GIB met February 8, 2017 Approved self-insurance funding structure and a regional

service model for State Group Health effective January 2018.

In addition: Uniform Benefits and the Access Plan benefits will be aligned. Income Continuation Insurance (ICI) Program – Redesign proposal

approved (concept for redesign). Medicare Plans – 3 of the Medicare Plans will be negotiated. ICI and Long-Term Disability Insurance Program – Administrative

services contract extension with Aetna Life Insurance Company approved

Proposed Health Insurance Plan Changes

A survey has been created to help manage questions that are asked and to assist in creating employee and administrator Frequently Asked Questions (FAQs).

As you get questions from employees, co-workers or if you have questions, please complete the survey.

Survey was emailed out February 24th.

Life Change Events

Gender change process in HRS and with ETF We are working with ETF to define a process for UW System

using the procedure ETF has provided. Details will be shared as the process for UW System is finalized.

2018 Annual Benefits Enrollment (ABE)

Annual Benefits Enrollment (ABE) dates have not been finalized.

Department of Employee Trust Funds (ETF) will ask for review/approval of proposed dates at the May Group Insurance Board (GIB) meeting.

Dates will be communicated to institution administrators and employees as soon as we have more information.

State Group Health Plan - COB Newly eligible employees who enroll in State Group Health

need to provide Coordination of Benefits (COB) information. Information required if an employee is married and spouse has health

insurance coverage available to them.

In Self Service (eBenefits) employees will be asked to enter this information. Health Insurance Application / Change Form (ET-2301) also requests this information if enrollment is not done in Self Service.

Service Center will be auditing and contacting institutions to work with the employee if the information is not provided.

Employees will be defaulted to a non-High Deductible Health Plan (HDHP) if they do not provide COB information within 10 business days of the close of their newly eligible window.

Wellness

Wellness portal is wellwisconsin.staywell.com All employees covered under the State Group Health Plan

have access to the website and tools on the site. 1-3 individuals at each institution have additional access in

the Wellness portal to Champion Connection which provides information that employers can use to promote wellness. Toolkits to start healthy initiatives Program promotional materials Webinar brochures Statewide biometrics schedule including registration

numbersand actual participation numbers once site session is completed.

Life Insurance Plans Effective date reminder for the following life insurance plans:

State Group Life Insurance Individual & Family Group Life Insurance UW Employees, Inc. Life Insurance Accidental Death & Dismemberment Insurance

New Hire / Newly Eligible: Application must be received by the employer within 30 days of the event date. Coverage is effective the 1st of the month following 30 days after the new hire / newly eligible event

date. For new hires refer to the Enrollment Deadline Worksheet for exact dates and deadlines.

Status Change (i.e. gaining a dependent): Application must be received by the employer within 30 days of the event date. Coverage is effective the 1st of the month following 30 days after the date of event.

Evidence of Insurability (EOI): 1st of the month following approval of application.

End of Eligibility or End of Employment (includes termination or retirement): End of the month.

Voluntary Cancellation: End of the month following the month in which the application is received.

Employer Error If approved, coverage is effective 1st of the month following the date the employer error application

is received by the employer. Note: UW Employees, Inc. Life Insurance does not have an employer error provision.

AD&D after New Hire: First of the month following receipt of the application for new coverage, changes and cancellation.

Flexible Spending Accounts

Flexible Spending Account (FSA) 2016 Plan Year deadlines 2016 plan year was January 1, 2016 – December 31, 2016. Last day to incur expenses was December 31, 2016. Claims for 2016 must be submitted to TASC by March 31, 2017

(90 days after close of the plan year).

Up to $500 carry over for Healthcare FSA and Limited Purpose FSA (LPFSA). 2016 due dates do not apply to carry over balances.

COBRA - Notification

Medical continuation forms, including for Vision, Dental, EPIC Benefits+ and FSA, must be given to the employee if they have a qualifying life event where there is a coverage loss. Resources:

Institution must complete Continuation Forms for all applicable plans in which the employee is enrolled.

COBRA – Continuation

Terminated employees can continue coverage for up to 18 months from first of month following termination date.

For ex-spouses/ex-domestic partners and children who lose dependent status due to age coverage can be continued for up to 36 months.

COBRA – Sending Forms Completed Continuation Forms must be submitted ETF

within 60 days of the coverage end date or the date on the continuation notice, whichever is later.

When sending COBRA information - Terminations:

Completed forms should accompany termination letter.

Divorce/ending of a domestic partnership: Confirm ex-spouse/ex-domestic partner’s address with employee, ex-

spouse/ex-domestic partner, attorney. Send forms via US Mail.

Children who lose dependent status due to age: Service Center will send a letter and continuation form to the employee 90

days in advance of the dependent becoming ineligible due to age. A second letter will be sent at 30 days prior to loss of coverage date.

Deferred ICI Enrollment & LOA

Deferred Enrollment period was through March 1, 2017. Enrollment information sent to eligible employees in late

January. Deferred enrollment is effective April 1, 2017. Job Data will be updated March 31 – April 2. Enrollments can be entered in HRS starting April 3, complete by

April 5.

If an employee missed deferred coverage enrollment opportunity because they were on a LOA at the time, they must submit an ICI enrollment application within 60 days of return to work if they choose to apply for coverage.

Summer Prepay Deductions Policy Reminders: Summer Prepay Deductions are tied to the SPRING appointment. Benefit premiums are not deducted from Summer Service /

Summer Session.

Summer Prepays and WRS The summer prepay determination DOES NOT constitute an

expected change for the purposes of WRS. Evaluate for WRS once the contract is extended and accepted.

2017 Holidays

For 2017, Christmas Eve and New Year’s Eve are both floating holidays for 2017.

University Staff must use both Floating Holidays by the end of the 2017 calendar year.

TSA 403(b) Procedure for TSA Quick Enroll Form Submission Employee submits the completed form to the institution’s human

resources office. The institution’s human resources office enters the TSA deduction

amount as a Savings event in HRS. The institution’s human resources office either scans or faxes the

form to the TSA program staff. Scan and email to tlauber@uwsa.edu and copy

pachterberg@uwsa.edu or fax to (608) 265-9834, Attn: Terry Lauber or Pam Achterberg.

Institution’s human resources office places the original in the employee’s file.

TSA program staff will complete the enrollment process with the chosen TSA plan provider.
