Click Create new > Form. Log into you gmail account and open Google Docs.

In the form template that opens, you can add any questions and options you'd like.Here you may choose among: text, paragraph text, multiple choice, check boxes, list, scale, grid.

Click Email this form once you've finished adding your questions.Add the email addresses of the people to whom you want to send this form. Emails must be separated by comas.Click Send.

Another options is to embed or link your form into your blog or webpage.Embedding: More actions, embed.Linking: Copy the address in the address navigation bar in your navigator, then paste it in the place you want people to access it.
