INTRODUCTION
The Epworth/Deakin Centre for Clinical Nursing Research is a joint initiative between Deakin
University’s School of Nursing and Midwifery and Epworth HealthCare. The Centre was
established in 2004 to conduct collaborative clinical research and provide staff and students
the opportunity to engage in clinically focussed, multidisciplinary research and training across
acute, rehabilitation and community healthcare settings.
The Centre provides academic and professional leadership to staff and students and recognises
the importance of combining expertise from both industry and academia to inform the quality
and safety of healthcare delivery. A defining feature of the Centre is that it fosters a rich
diversity of activities in health services research with the aim of enhancing the research culture
at Epworth with benefits for staff, academics, students and patients.
Objectives
The four key objectives of the Centre are to:
1. Pursue a program of research that supports the core clinical activities of Epworth
HealthCare and is situated within the Quality and Patient Safety (QPS) research cluster
of the Faculty of Health, Deakin University. Our three overarching research priorities
are:
a. Patient engagement and participation in healthcare
b. Patient experiences of care and recovery
c. Clinical support for delivery of high quality, safe care.
2. Support Quality and Patient Safety activities at Epworth HealthCare through research
translation.
3. Provide leadership and mentorship for the professional development of clinical and
academic staff through research training and education.
4. Establish a public profile of the Centre that promotes its activities and research
outcomes and highlights the expertise of its researchers as leaders in their field.

The Epworth/Deakin Centre for Clinical Nursing Research is directed by Professor Mari Botti.
As Chair in Nursing she reports to the Head of the School of Nursing and Midwifery at Deakin
University and the Chief Executive of Epworth Hospital.

To investigate the potential influence clinicians have on increasing physical activity levels among prostate cancer survivors, by referring cancer survivors to a physical activity program, with expert involvement from exercise physiologists. To test the hypothesis that excessive caregiver burden is modifiable through a comprehensive low cost intervention that includes supportive care and telephone support. To undertake a pilot study to test whether the intervention: reduces caregiver burden; improves psychological health (depression); increases ability to navigate health services; increases health literacy (ability to understand, access and utilise health services); improves quality of life; and is cost effective within a formal health economic evaluation.

To estimate the ability of an innovative, convenient and low cost screening device “DeVeTect” to detect lower limb vascular abnormality, which includes DVT. If the screening device meets expectations, it can have immediate implications for improved clinical management of, and outcomes for, various patient groups susceptible to DVT. This study will test the device in patients undergoing knee arthroplasty, a population of patients known to be at high risk for DVT and likely to receive benefit if successful.

Despite their central role in health care delivery, intelligent systems have rarely been designed around nurses’ operational needs. SmartWard, an Australian SME (small medium enterprise) has developed such a system. Our study examines the feasibility of this intelligent care support tool to assist nursing care at Epworth. It will be tested in a simulated environment, thereby also enabling the development of protocols for testing IT solutions in the SIM Centre.

Patient Outcomes after Open and Minimally Invasive Surgery for Prostate
Cancer: Longitudinal Study
In Australia, prostate cancer is the most commonly diagnosed cancer in males and the second most
common cause of cancer deaths in men. The treatment for localised prostate cancer is most often
prostatectomy surgery. Prostatectomy surgery includes traditional open radical prostatectomy and
minimally invasive surgery including robotic-assisted prostatectomy surgery. The physiological,
functional, psychosocial and cognitive outcomes associated with the long term trajectory of
recovery have not been investigated adequately, nor has there been a prospective, comparative
evaluation of the outcome associated with different surgical modalities. This longitudinal,
comparative study examines patient outcomes after open and minimally invasive surgery for
localised prostate cancer.
A unique collaborative research team consisting of surgeons, academics and specialist urology
nurses was established to undertake this study funded by a three year grant from the Prostate
Cancer Foundation of Australia. This research is significant because little is known of the trajectory
of patient recovery after prostatectomy surgery within the setting of an ageing Australia and
advances in technology. The findings will address gaps in research, inform patients about their long
term recovery, and provide evidence for optimal care of patients who undergo prostatectomy for
localised cancer irrespective of the surgical modality.
Reducing Carer Burden and Depression among Caregivers of Cancer Patients
Family or friend caregivers of cancer patients provide extended and unrecognised support to
hundreds of thousands of Australians each year. Up to 70% of caregivers of cancer patients report
depression and one-third report excessive burden. We have developed clear mechanisms by which
the highly successful Cancer Council Victoria’s Cancer Helpline can potentially reduce carer burden
and the psychosocial impact of caring. The aim of the study is to reduce caregiver burden and
depression with improvements in health service navigation, health literacy and quality of life,
underpinned by a formal economic evaluation to assess the cost-effectiveness of the intervention
program.
The design of the study is a single, blind, multi-centred randomised controlled trial to determine the
efficacy and cost-effectiveness of the intervention for caregivers of people who are receiving chemo
or radio therapy for newly diagnosed cancer. Caregivers are referred to the Cancer Helpline from 4

Victorian health services (Epworth HealthCare, Eastern and Barwon Health and Peter MacCallum
Cancer Centre-Box Hill). A Helpline nurse telephones caregivers three times during the treatment
cycle and addresses: psychological distress; health literacy; general health; family support; financial
impact and provides practical advice. The Cancer Helpline nurse also assesses carer distress with
referrals back to their General Practitioner for psychological follow up.
Translation of evidence into pain management practices in acute care
environments
A major focus of the drive to improve the quality and safety of health care is to standardise
care processes to ensure patients receive care based on best available evidence and do not
experience variations in the quality of care. Management of post surgical pain is a care process
known to be highly variable and inadequate. In Australia, over 40% of patients experience
significant pain and hence, unnecessary suffering and increased associated risk. In partnership
with clinicians, the aim is to develop, implement and evaluate an improved method of treating
pain using a clinical decision support system based on best available evidence for managing
postoperative pain. The method will be tested in three sites to assess external applicability.
Patient Participation in Healthcare
Patient participation is central to initiatives to increase the safety and quality of health care
worldwide. Little is known about the key determinants for the success of patient participation,
in particular, the effect of patient-related factors such as patients’ preference for participation
and context-related factors associated with the culture of care, policies and legislation relating
to patient participation. This study has been designed to extend the knowledge gained from
doctoral work. The aim of this pilot study is to collect baseline, cohort data of the patient and
context influences on patient participation in symptom management in acute cancer care
environments. Data will be collected in Australia, China and Thailand. The data obtained from
this pilot study will be used as the foundation for future exploratory and intervention research.
Evaluation of the acceptability and fidelity of an electronic Clinical Decision
Support and Documentation System
This research program is focused on trying to understand the general lack of success, in terms
of uptake and effectiveness, of clinical decision support and documentation systems in acute
care environments and through this research develop more effective approaches to design and
implementation.

A sequence of studies has been undertaken through a long-term research collaboration
between Deakin University and SmartWardTM. Structured engagement with potential users
(clinicians, administrators and quality and safety personnel) and ongoing research has
underpinned a 2 year design process that has incorporated the investigation of the
acceptability of a decision support and documentation solution (SmartWardTM), preclinical
evaluation of the usability of the solution, and a hospital environmental study to investigate
the fidelity of the new system to the process and function of care in real work contexts.
