Eat Smart, Move More, Prevent Diabetes IS:
• A lifestyle change program based upon strategies proven to prevent or
delay the type 2 diabetes.
• A 12-month program delivered in an interactive, real-time, online
format using distance-education technology.
• Divided into two phases with 16 lessons in Phase 1 and 8 lessons in
Phase II.
• A CDC approved program for diabetes prevention.
Program Participants WILL:
• Attend classes online in an interactive format from their computer or
mobile device.
• Be empowered to make healthy eating and physical activity choices.
• Learn strategies to achieve and maintain a healthy weight.
• Identify ways to manage stress.
• Connect with others working on similar goals.
• Receive personalized support outside of class from their instructor.

their weekly progress as well as communicate with their instructor.

Registration
• Registration is available - esmmpreventdiabetes.com.
• Prediabetes screening prior to allowing someone to register.
• Lunchtime and evening class options available during each class series.
• New series will begin monthly (with exception of July, November, December).
