** Aim of the event **Provide a platform for CCGs who are delivering new ways of working across dementia within primary care to showcase their proposals. Bring together North of England colleagues to discuss models of managing dementia within primary care.A peer to peer learning opportunity in dementia pathways and post diagnostic support

Each CCG will develop and take away an action plan to improve dementia diagnosis and post diagnostic support, utilising the experiences and learning of speakers and presenters to consider opportunities to launch a local pilot
