Document Must Include:
A. Pay Stub
DC tax withholding for current tax year
Issued within the past 45 days
B. Unexpired letter confirming Financial Assis-
tance from the DC Government
Ex. TANF, Medicaid, DC Alliance, Supplemental Security Income
Issued within the past 12 months
C. Certified copy of DC Tax Form D40
Certified by DC Office of Tax and Revenue
Evidence of payment of DC taxes for prior tax
year
D. Embassy Letter to prove the applicant lives on
embassy property
Issued within the past 12 months
Embassy address
Official embassy seal

IF YOU CANNOT provide one of the documents above, you may bring
TWO (2) Originals of the following:
E. Unexpired DC motor vehicle registration
F. Unexpired lease or rental agreement and 1
receipt
Receipt of payment of rent within the past 2

H. One utility bill and 1 receipt (Only gas, electric, and water bills are acceptable)
Receipt of payment for the utility bill within the
