The Columbus District Golf Association will send $5 per player fromeach entry fee to the Capital Campaign for the new Evans ScholarHouse at the Ohio State University. The new house, located at 52East 14th Avenue is named the Hamilton House in honor of Earl“Bones” Hamilton. $5 per player of the entry fee quali es as acharitable donation for federal income tax purposes.

EVANS SCHOLARSHIPEvans Scholars Foundation administers the nation’s largest privatelyfunded college scholarship program. Since 1930, over 8,500deserving young caddies have earned their college degrees throughEvans Scholarships. Today, more than 820 caddies attend collegeeach year as Evans Scholars. The Evans Scholarship was founded bygolf great Charles “Chick” Evans Jr. (1890-1979).

ELIGIBILITYOne of the two team members must be a full member of a CDGA Af liated Club.CDGA GHIN # must be on application. Non CDGA Members must have an active GHIN #with at least 10 scores posted in the past 12 months.

RULESUSGA rules will govern all play except for local rules approved by CDGA.FORMATField limited to 60 teams. Scramble, 18 Holes stroke play (40% of combined

sloped handicap will be used). It is a condition of this competition that the Course Handicaps(adjusted) of the members of a side not differ by more than 10 (ten) strokes.

ENTRYEntry is subject to approval or rejection at any time by CDGA Tournament Committee.No practice round is included in entry.

ENTRY FEE$150 per team. Entry includes cart, range and buffet lunch. Fee will not berefunded after deadline unless team submits a written medical excuse. Riviera GC membersmay enter this event for $120 (20% discount as a host club member). Only one team memberhas to be from host club.

DEADLINEMay 3, 2012, 4 p.m. (or when eld lls). No Tee Entries. No Fax, phone or e-mailentries.

CARTSGolf carts and caddies are permitted. Players may carry clubs if host club permits.CADDIESThe following constitutes a condition of the competition and is authorized by

Appendix I, Part C, Section 3 of the Rules of Golf. “A player is prohibited from having aparent, guardian or another player in the same competition serve as his caddie during thestipulated round.”

TEE TIMESRequested tee times can not be guaranteed. Copies of starting times will be sent toCDGA Pro Shops approximately one week in advance of event. Contestants may also checkOHIOGOLF.ORG for tee times..
