Checklist for your annual examHERE’S HOW TO MAKE THE MOST OF YOUR ANNUAL EXAM:Ask about screenings or vaccinations. When making your appointment, find out if you’re due for any shots or tests. You may need a tetanus shot, Pap test, mammogram, cancer or other screening. Knowing in advance can help you prepare mentally and physically — especially if you need to follow specific test instructions, such as fasting for a cholesterol test.
Review your family tree. Have new diseases or conditions occurred among close relatives since your last visit? Share this information with your doctor. If you have a family history of heart disease,
stroke, diabetes or cancer, your doctor may emphasize prevention and/or recommend screening tests.
Note health changes. Have you noticed lumps, dizziness, pain or other problems? Are you experiencing depression, anxiety or sleep problems? Write this information down and when you noticed the changes to help your doctor better treat you.
Know your visit is confidential. Your doctor may ask detailed questions. It can be difficult for patients to mention what may seem like an embarrassing health issue, such as urinary incontinence, erectile dysfunction and domestic violence for

It’s essential to prepare and keep your thoughts organized. This helps your doctor make better use of limited time. And you’ll have a better experience as well. “An annual physical helps your doctor determine your overall health status. More importantly, it’s a chance to check for possible diseases so they can be treated before they get too serious,” says Dr. Aislinn Rogalla, Family Physician at Grand Itasca. “It’s also a chance to identify risk factors that could lead to future health problems and offer advice on how to manage them.”

various reasons. But it’s your doctor’s job to keep you healthy; and no health concern is embarrassing to them.
Write down questions. Take your list of concerns and questions with you. And bring a pad and pen to jot down notes as the doctor answers them. When you’re in the doctor’s office, it’s often hard to remember everything. If you or a loved one struggles with hearing, understanding or memory, ask for help. “Often times, patients have follow-up questions. If you have questions after your visit, call the office. A nurse can check with your doctor and call you back. Or you can ask if your health professional has an online health portal you can use to send questions,” Dr. Rogalla shares.
Think ahead. Set your expectations now for the next year, whether that includes losing weight, trying to get pregnant, starting an exercise routine, quitting smoking or other goals. Discuss these goals with your doctor. He or she may make recommendations, or refer you to helpful resources and specialists. “Your doctor is a valuable asset in helping you reach your goals and remain as healthy as you can be,” says Dr. Rogalla.

WHAT TO BRING TO YOUR ANNUAL EXAM1. A list of medications.Bring a list of your current medications (including herbal supplements,
vitamins and over-the-counter medications).
2. Results of medical tests.If you’ve had any medical tests since your last physical, bring a copy
of the results. If you’ve seen any specialists, bring their information as
well. Your primary care provider will help coordinate your care.
3. A current list of your immunizations.Bring a list of your immunizations so your PCP can ensure you are
up-to-date.
4. A notebook or paper and pen.Many people get nervous during their visit and forget their questions.
Write everything down.
5. A friend or family member.It’s helpful to have an extra set of ears to make sure you get the
information you need.
