DOCUMENTS. ALL ARCHITECTURAL AND STRUCTURAL MODIFICATIONS HAVE BEEN CLEARLY DEFINED IN THESE
DOCUMENTS.
THE DRAWINGS AND ANNOTATIONS INCLUDED HEREIN, ESTABLISH THE CRITERIA FOR THE SELECTIVE DEMOLITION,
STRUCTURAL, ARCHITECTURAL, MEP ENGINEERING ASSOCIATED WITH THE REMODELING OF THE EXISTING TWO
STORY, MULTI-FAMILY RESIDENCE LOCATED AT 4686 DAVIS RD, LAKE WORTH, FL.
THE PROJECT IS DEFINED AND WORK SHALL BE PROVIDED BASED ON THE CONTRACT DOCUMENTS PROVIDED
HEREIN. THE CONTRACT DOCUMENTS ARE COMPRISED OF THESE WORKING DRAWINGS, PLAN NOTES AND
SPECIFICATIONS. THE GENERAL CONTRACTOR SHALL PROVIDE ALL LABOR AND MATERIALS AND COMPLETE ALL
NEW WORK BASED ON A COMPLETE SET OF CONTRACT DOCUMENTS.
TO THE BEST OF OUR KNOWLEDGE, THE STRUCTURAL DETAILING AND SPECIFICATIONS COMPLY WITH THE
APPLICABLE REQUIREMENTS OF THE GOVERNING BUILDING CODE.
CONSTRUCTION IS TO COMPLY WITH THE REQUIREMENTS OF THE GOVERNING BUILDING CODE AND ALL OTHER
APPLICABLE FEDERAL, STATE AND LOCAL CODES, STANDARDS, REGULATIONS AND LAWS.
THE CONTRACTOR SHALL PROTECT ADJACENT PROPERTY, HIS OWN WORK AND THE PUBLIC FROM HARM. THE
CONTRACTOR IS SOLELY RESPONSIBLE FOR CONSTRUCTION MEANS AND METHODS, AND JOBSITE SAFETY
INCLUDING ALL OSHA REQUIREMENTS.
FIELD VERIFY ALL OPENING DIMENSIONS:
IT SHALL BE THE RESPONSIBILITY OF THE OF THE GENERAL CONTRACTOR TO VERIFY THE EXACT OPENING
DIMENSIONS AND TO ESTABLISH THE EXACT WINDOW AND DOORS DIMENSIONS THAT ARE REQUIRED TO
ACCOMMODATE THESE OPENINGS. THE DIMENSIONS SHOWN IN THESE DOCUMENTS ARE FOR REFERENCE
PURPOSES ONLY AND SHALL NOT BE CONSTRUED AS THE FINAL DIMENSIONS OF THE WINDOW AND DOOR
PRODUCTS TO BE ORDERED.
THE GENERAL CONTRACTOR SHALL RECEIVE MANUFACTURER'S SHOP DRAWINGS LISTING ALL SIZES, FUNCTION AND
FEATURES AS ASSOCIATED WITH THESE WINDOW AND DOOR PRODUCTS. THESE SHOP DRAWINGS SHALL BE
REVIEWED AND APPROVED BY THE GENERAL CONTRACTOR PRIOR PURCHASING AND ORDERING ANY WINDOW AND
DOOR PRODUCTS.
GENERAL CODE REQUIREMENTS:
THIS RENOVATION PROJECT WILL BE DESIGNED UNDER THE CURRENT FLORIDA BUILDING CODE FOR FLORIDA. THIS
CODE IS THE FLORIDA BUILDING CODE 2007 EDITION WITH FLORIDA AMENDMENTS, AND AS INTERPRETED BY THE
LOCAL AUTHORITY HAVING JURISDICTION OVER THIS PROJECT.
· THE RENOVATIONS WILL BE DESIGNED TO COMPLY WITH THE FLORIDA EXISTING BUILDING CODE - BUILDING -
2017 EDITION.
· THE HVAC SYSTEMS WILL BE DESIGNED TO COMPLY WITH THE FLORIDA BUILDING CODE - MECHANICAL- 2017
EDITION.
· THE ELECTRICAL SYSTEMS WILL BE DESIGNED TO COMPLY WITH THE NATIONAL ELECTRICAL CODE (NEC) -
CURRENT EDITION.
GENERAL DESIGN CRITERIA:
GENERAL BUILDING, STRUCTURES AND PARTS THEREOF SHALL BE DESIGNED AND CONSTRUCTED IN ACCORDANCE
WITH STRENGTH DESIGN, LOAD AND RESISTANCE FACTOR DESIGN, ALLOWANCE STRESS DESIGN, EMPIRICAL
DESIGN OR CONVENTIONAL CONSTRUCTION METHODS.
THE MODIFICATIONS TO THE BUILDING STRUCTURE HAVE BEEN DESIGNED IN ACCORDANCE WITH THE FLORIDA
BUILDING CODE 2017 EDITION WITH SUPPLEMENTS USING THE FOLLOWING DESIGN CRITERIA:

00.1 ABATEMENT AND WASTE MANAGEMENT
ASBESTOS CONTAINING MATERIALS WERE FOUND IN THIS EXISTING RESIDENTIAL PROPERTY.
GAUDET ASSOCIATES, INC., A LICENSED FLORIDA ASBESTOS BUSINESS (ZA#OOOOO11), WAS
RETAINED TO PERFORM A PRE-RENOVATION ASBESTOS MATERIAL IDENTIFICATION SURVEY FOR A
FOUR APARTMENTS AT 4680 DAVIS ROAD, IN LAKE WORTH FLORIDA.
THE SURVEY ENCOMPASSED APARTMENTS 4686, 4692, 4694 AND 4690. THE STRUCTURES ARE
CONSTRUCTED OF CONCRETE BLOCK ON CONCRETE SLABS WITH ASPHALT SHINGLE ROOFS. THE
INTERIOR CONSISTS
OF POPCORN CEILINGS, DRYWALL AND FLOOR TILE. THE APARTMENTS ARE SLATED TO BE RENOVATED.
GAUDET ASSOCIATES, INC. SCHEDULED AN EPA CERTIFIED BUILDING INSPECTOR, MR. MICHAEL
MCGOVERN TO PERFORM THE SURVEY ON OCTOBER 24,2017 AND JUNE 6, 2018. THE PURPOSE OF THE
SURVEY WAS TO IDENTIFY, BULK SAMPLE AND QUANTIFY SUSPECT ASBESTOS CONTAINING BUILDING
MATERIALS IN THOSE AREAS ACCESSIBLE TO THE INSPECTOR. DURING THE INITIAL INSPECTION, THE
INSPECTOR COLLECTED A TOTAL OF TWENTY-EIGHT (28) SAMPLES. DURING THE SUBSEQUENT
INSPECTION, AN ADDITIONAL THIRTEEN (13) SAMPLES COLLECTED. THE INSPECTOR COLLECTED A TOTAL
OF FORTH-ONE (41) SAMPLES.
THESE SAMPLES WERE ANALYZED BY A NATIONAL VOLUNTARY LABORATORY ACCREDITATION
PROGRAM (NVLAP) ACCREDITED LABORATORY USING POLARIZED LIGHT MICROSCOPY (PLM).
THE ENVIRONMENTAL PROTECTION AGENCY'S (EPA) DEFINITION OF AN ASBESTOS-CONTAINING
MATERIAL IS ONE THAT IS GREATER THAN ONE (1%) PERCENT ASBESTOS. THREE (3) OF THE SAMPLES
COLLECTED ON JUNE 6, 2018 TESTED POSITIVE FOR ASBESTOS CONTENT.
THE ASBESTOS CONTAINING MATERIALS WERE FOUND IN THE SINK UNDERCOATING (WHITE) IN THE
KITCHEN OF APARTMENTS 4686, 4694 & 4692. APPROXIMATE QUANTITY, THREE (3) DOUBLE SINKS.
SINK UNDERCOAT (WHITE)
THE SINK UNDERCOAT (WHITE) CONTAINS FIVE (5%) PERCENT CHYSOLTILE ASBESTOS AND IS
NON-FRIABLE. THIS MATERIAL IS LOCATED IN THE KITCHENS OF OF APARTMENTS 4686, 4694 & 4692.
DEMOLITION
NESHAP CATEGORY I NON-FRIABLE MATERIALS MAY BE DEMOLISHED WITH THE STRUCTURE, USING
ADEQUATE CONTROLS. THE DEMOLITION CONTRACTOR SHALL BE MADE AWARE OF THE
ASBESTOS-CONTAINING MATERIALS AND SHALL EXERCISE ADEQUATE CONTROL TECHNIQUES (WET
METHODS, ETC.). ANY EXCEPTIONS TO THESE GUIDELINES SHALL BE REQUESTED THROUGH AND
APPROVED BY THE COMMUNITY PARTNERS PRIOR TO THE REMOVAL. DEMOLITION WORK SHOULD BE
MONITORED BY A FLAC TO ENSURE PROPER CONTROL MEASURES AND WASTE DISPOSAL. THIS IS THE
RESPONSIBILITY OF THE AGENCY (OR MUNICIPALITY, AS APPLICABLE).
IF SUSPECT MATERIALS ARE DISCOVERED THAT WERE NOT PREVIOUSLY SAMPLED AND IDENTIFIED IN
THE SURVEY, STOP ALL WORK THAT WILL DISTURB THESE MATERIALS AND IMMEDIATELY NOTIFY THE
COMMUNITY PARTNERS.
A NESHAP FORM MUST BE PREPARED BY THE CONTRACTOR AND SUBMITTED TO THE PALM BEACH
COUNTY HEALTH DEPARTMENT AT LEAST TEN (10) WORKING DAYS PRIOR TO THE DEMOLITION.
THE NESHAP FORM IS AVAILABLE ONLINE THROUGH THE FLORIDA DEPARTMENT OF ENVIRONMENTAL
REGULATIONS. THE NOTIFICATION SHALL BE SENT TO THE ADDRESS SHOWN BELOW. A COPY SHALL BE
INCLUDED IN THE POST JOB DOCUMENTATION SUBMITTED TO THE COMMUNITY PARTNERS ALL FEES

THE CONTRACTOR SHALL FIELD VERIFY THE FINAL AS BUILT CONDITIONS, MEASUREMENT AND
PROVIDE A SHOP DRAWING ILLUSTRATING THE LAYOUT OF PROPOSED CABINET WORK. THIS
DRAWING SHALL BE ISSUED TO THE ARCHITECT FOR REVIEW, PRIOR TO THE PURCHASE AND
INSTALLATION OF ANY NEW KITCHEN CABINETS AND COUNTERTOPS.
KITCHEN CABINETS AND COUNTERTOPS: THESE PLANS ILLUSTRATE THE REQUIRED LAYOUT AND

SATISFY THE 2017 FLORIDA BUILDING CODE FOR BOTH IMPACT RESISTANCE AND PRESSURE
REQUIREMENTS.
FURNISH AND INSTALL NEW ALUMINUM IMPACT RATED WINDOWS IN THE LOCATIONS AS SHOWN ON
THE PLANS. INSTALL A SUFFICIENT BUCK PER THE NOA AND THE FLORIDA BUILDING CODE PRODUCT
DATA.
NEW WINDOWS SHALL BE CGI SENTINEL IMPACT RATED WITH THERMAL PANE AND LOW E AND
BRONZE TINTING.
THE FUNCTION AND OPERATION OF THE WINDOWS SHALL AS SHOWN ON THE PLANS AND WINDOW
AND DOOR SCHEDULE.

FURNISH AND INSTALL SHOWER DIVERTER, FIXTURES AND SHOWERHEAD WITH A
WASHERLESS SINGLE HANDLE DIVERTER ASSEMBLY FAUCET VALVES WITH SCREWDRIVER
STOPS, MOEN CHATEAU OR EQUAL. INSTALL NEW SPOUT AND MAXIMUM 2.0 GPM WATER
SAVING SHOWER HEAD.
15.3 ELECTRICAL WATER HEATERS
REMOVE EXISTING WATER HEATER AND DISPOSE OF PROPERLY. PROVIDE A NEW ENERGY
EFFICIENT, 50 GALLON GAS WATER HEATER ALL RELATED PIPING AND VALVING. THE WATER
HEATER SHALL HAVE TWO (2) HEATING ELEMENTS AND CARRY A TWELVE YEAR
MANUFACTURERS WARRANTY. INSTALL NEW COPPER PIPING, SHUT-OFF VALVES AND NEW
TEMPERATURE PRESSURE RELEASE
VALVES, WATER SUPPLY VALVES, AND PRESSURE VALVE PIPED IN 3/4" COPPER PIPE TO THE
EXTERIOR.
NOTE: CONTRACTOR SHOULD FIELD VERIFY THE SPACE CLEARANCE OF THE DESIGNATED
AREA FOR THE NEW WATER HEATER TO BE INSTALLED. NOTIFY ARCHITECT BEFORE
ORDERING OR PURCHASING TO ENSURE THE RIGHT SIZE OF WATER HEATER IF ANY
CONSTRAINTS WITHIN THE SPACE PROVIDED.
