Logic Model Implementation & Evaluation
1. Capture baseline data from seniors using survey pre workshop and pre referral
2. Follow up with post survey after workshop to capture changes in beliefs and after referral/intervention (focus groups)
3. Follow up with staff, volunteers, other champions weekly to assure they feel supported
4. Visit with the plan monthly5. Report findings monthly stay the course6. *Celebrate successes & adjust as needed
