Health Disaster Exercise • To Review:

You may be thinking…….

Why Anthrax?

Anthrax Attacks 2001- 7 letters mailed in 4 states

Inhalation Anthrax

• Onset of symptoms: – 2 days up to 60 days post-exposure (may be

1 day) • Antibiotics:

– most effective before onset of symptoms – Mortality 90-95% without treatment

• LD50 about 8000 spores – Infective dose probably less than 10 spores in susceptible

persons – Gram positive, rod shaped, spore forming baccilis

The CDC and federal

government have identified threats that

could require immediate mass distribution of medications in our

communities. These include:

Regional Exercise Scenario • Thursday, 11/12: Los Angeles: letter with

white powder received at STAR Academy in Los Angeles County.

• Friday, 11/13: ER MD in LA reports potential anthrax in patient with group of small blisters that itch. Patient has a painless skin sore (ulcer) with a black center.

On Monday, November 16, 2015, Biowatch samples tested by Polymerase Chain Reaction (PCR) are

positive (+) for Bacillus anthracis (the causing agent for anthrax) from the majority of Biowatch sites in

Southern California.

Status Report

Status Reports from Hospitals: Reddinet Use in Exercise

• “This is a Drill” in all messages • 0900-1100

– “The 2015 medical health exercise has begun please answer the Reddinet polls.”

– Hospital Command Center standard poll – Status poll for anthrax suspect patients, surge of worried well,

decon, and mass prophylaxis status

• Reddinet messages with resource requests must be followed up with a written signed request.

• PHD DOC medical health branch staff will request a follow up resource request

Prophylaxis Delivery

• Staffing to conduct the prophy screening

• Location to conduct the screening

• Tables/chairs/pens or clipboards

• Copies of the screening form (English/Spanish)

• Copies of the algorithm • Options:

– Email both forms out to staff or residents in advance

SCREENING CHILDREN

If a child or adult is allergic to Doxycyline and cannot swallow pills they must be sent to medical consultation.

SCREENING EXAMPLES

1. Holly Lolipop Age: 2 allergic to Cipro cannot swallow pills

2. Jose Doxinon Age: 4 allergic to Doxy cannot swallow pills

3. Rose Klencher Age:92 no allergies cannot swallow pills

4. Lilly Pensquno Age: 54 allergic to both Cipro and Doxy

Number of Courses Received= How many bottles of faux antibiotics did you receive? - 1 Case = 100 bottles - 1 Pallet= 9,600 bottles

Number of Courses Dispensed - Number of staff/residents

that received Doxy or Cipro Count the screening forms to fill in these numbers.

• We will be conducting video conferencing during the exercise to receive a status report from the hospital HCC, the warehouse, and PHD DOC.

• We have used video conferencing with your facilities in the past and that there have been some issues.

• We can now use Skype for Business which can make things easier.

• Is your facility willing to participate via video conferencing/Skype?

• If so who can our IT contact to discuss and test in advance of the exercise?

have press or media at your site. – Media has already begun publicizing mass

vaccination flu clinics. – Media may want to film delivery to hospital sites.
