Vo-tech requests should submit 7.0 credit registration – will need to complete a separate Vo-Tech application if first year or changing program – indicate program name and course number on registration form

Cooperative Education requests (seniors only) - Cooperative Employment Theory (#0620 or #5620) on schedule and complete a 4.0 credit schedule - indicate “Co-op” as a special schedule request

Indicate if requesting Lab periods opposite a 2 or 3 day class

Review course descriptions to make sure you meet all prerequisites/corequisites

Choose courses supporting future plans and a challenging program through graduation – review Math, Social Studies, and Science sequence charts

Obtain teacher signature for all advanced level core courses, World Languages, and electives requiring applications/auditions

form Must complete the online Cyber Summer School application

All cyber grades are listed on the transcript and report card. Acceleration credit will receive a letter grade/percentage and

will calculate into the QPA for the upcoming school year Credit Recovery will receive a Pass/Fail grade, will not

calculate into the QPA, and will be listed on the current year transcript

Substitute course section must be filled in completely and must be different than the original requests

Student and Parent/Guardian must sign the completed form

Course request changes can be made through the end of the school year June 5, 2015
