State Grant Form
Beginning in the 2009-10 award year, the Pennsylvania State Grant program will collect the non-FAFSA data elements needed to calculate the State Grant award through an online process. The new electronic “State Grant Form” (SGF) will be available for students to complete following the completion of their 2009-10 FAFSA.

The SGF will be housed on Account Access. If a student email address was provided on the FAFSA then the student will receive an email directing them to Account Access to complete the SGF. For dependent students whose parent provide an email address, the parent will be emailed as well. If no email address was provided then a letter will be mailed prompting them to visit Account Access.

When the student clicks on the “Print Your State Grant Form,” the “Applicant's Rights, Responsibilities, Use of Social Security Number, and Certification for the Pennsylvania State Grant Program” and the information provided on Account Access will be displayed and printed.

The student must sign the last page of the State Grant Form along with a parent if the student is dependent. The SGF is mailed to PHEAA and receipt of this signed document will complete the State Grant application process for the student.

Status Notice
In the past, all first time applicants received a State Grant Status Notice which included not only the non-FAFSA items needed for State Grant award determination but also listed all of the FAFSA questions in case changes needed to be made. This form was quite long and generated many questions.

Status Notice
If corrections to FAFSA elements are needed, students will be encouraged to correct them on the FAFSA website as opposed to using a PHEAA’s Status Notice form. This will create more consistency of ISIR information held by schools and PHEAA.

Status Notice
There will be some instances when a Status Notice will be required from the student. This would be primarily in cases concerning dependency status, for example, if the school made a student independent through professional judgment on the first ISIR transaction.

We are moving towards increasing use of email as our first line of communication to students. They will be urged to use Account Access and to be attentive to email received from:

for important information and requests.

For students who do not respond to email communications or who do not complete the online SGF, paper mailing will eventually be sent to them.
