SCHEDULING YOUR CAPSTONE (ITP CHECKLIST – DD FORM 2958)

REQUIRED CAPSTONE ITEMS (DD Form 2958)

1. PRESEPARATION COUNSELING CHECKLIST Completed DD Form 2648

2. Individual Transition Plan (ITP) – Completed, as applicable. (Minimum – Pages 1-9, and applicable section to what you are doing when leaving the military: Employment, Education, Career Technical, or Entrepreneurship).

3. Completed job application, resume/references, or offer letter

Resume Completed in TGPS

4. Proof of ebenefits registration https://www.ebenefits.va.gov/ebenefits-portal/ebenefits.portal

5. Completed 12 month post separation budget (electronic or hardcopy)

6. Completed GAP ANALYSIS from MOC crosswalk - Day 1 TAP

7. Educational acceptance letter If applicable

8. DOL Gold Card

The Transition counselor will also discuss additional information with the client, to include continuum of military service, licensure/certification/apprenticeship, additional TGPS tracks, exemptions, housing, transportation, education counseling, and assessment tools.Please contact the FFSC or A&FRC to talk to a Transition Counselor if you have specific questions or clarification.
